Toothpaste contains several ingredients that may cause problems for some individuals, although most mainstream brands such as Colgate and Pepsodent use these ingredients in regulated, safe amounts approved by dental associations like the ADA (American Dental Association) and IDA (Indian Dental Association). For the majority of people, these products are safe when used as directed. However, certain ingredients have been discussed in relation to irritation, allergies, or potential long-term concerns.

One commonly discussed ingredient is Sodium Lauryl Sulfate (SLS), a foaming agent that helps spread toothpaste and improve cleaning. In sensitive individuals, SLS may cause mouth irritation, canker sores, gum inflammation, or dryness. Some claims have linked SLS to cancer, but scientific evidence supporting this is weak and not conclusive. People who frequently experience mouth ulcers often prefer SLS-free toothpaste.

Fluoride (such as sodium fluoride or stannous fluoride) is considered the most effective ingredient for preventing cavities. It strengthens enamel and reduces tooth decay. However, excessive ingestion—especially in young children—can cause dental fluorosis, which appears as white or brown spots on teeth. At very high levels, fluoride can be toxic, but the concentration used in standard toothpaste is considered safe. Children should use only a pea-sized amount and avoid swallowing it.

Triclosan was previously used as an antibacterial agent to reduce plaque and gum disease. Concerns about possible endocrine (hormonal) disruption and antibiotic resistance led many countries and manufacturers to reduce or eliminate its use. It is now uncommon in most modern toothpastes.

Propylene Glycol is used to maintain moisture and prevent toothpaste from drying out. While large systemic exposure may affect organs like the kidneys or nervous system, the small amount present in toothpaste is generally regarded as safe. Similarly, parabens, used as preservatives to prevent bacterial growth, have raised concerns due to possible endocrine-disrupting effects in some studies, but evidence of harm at typical exposure levels remains limited.

Artificial sweeteners such as saccharin or aspartame, along with artificial colors, are added to improve taste and appearance. These may trigger allergies or sensitivities in certain individuals, though they are approved for use in regulated quantities. There have also been occasional reports of trace heavy metal contamination (such as lead or arsenic) in some natural or mineral-based toothpastes containing ingredients like bentonite clay or calcium carbonate. These metals are not intentionally added; contamination can occur from raw materials. Reputable brands are expected to keep such contaminants within regulatory safety limits.

Regarding mouthwash, some ingredients may cause side effects, particularly with long-term or excessive use. The most debated component is alcohol (ethanol), found in some formulations at concentrations of 20–26%. Alcohol is effective at killing bacteria, but it can cause dry mouth, irritation, and may trigger ulcers. Dry mouth itself can increase bad breath and dental problems. Some studies have explored a possible association between long-term heavy use of alcohol-based mouthwash and oral or head-and-neck cancer due to the production of acetaldehyde, a known carcinogen; however, the evidence remains mixed and not definitive. Many experts recommend alcohol-free versions for daily use, especially for people with dry mouth or sensitivity.

Chlorhexidine (gluconate or digluconate) is a strong antiseptic commonly prescribed for short-term use (usually 1–2 weeks) to treat gum infections or after dental procedures. Prolonged use can cause brown staining of teeth and tongue, altered taste sensation, and disruption of the normal oral microbiome. It should only be used under professional guidance.

Triclosan, previously found in some mouthwashes, has largely been removed due to hormonal and antibiotic resistance concerns. Other components such as SLS or similar surfactants, artificial sweeteners, artificial dyes, propylene glycol, and parabens may cause irritation or allergic reactions in sensitive individuals.

Popular brands vary in formulation. For example, traditional variants of Listerine contain alcohol and essential oils such as thymol, eucalyptol, menthol, and methyl salicylate, which can cause burning sensations in some users. Colgate Plax has both alcohol and alcohol-free variants depending on the version. Betadine gargle contains povidone-iodine, which is effective against infections but not intended for long-term daily use. Products like Hexidine or Clohex contain chlorhexidine and are usually recommended for short therapeutic courses. Alcohol-free options from brands such as Colgate, Sensodyne, Himalaya, Patanjali, or other herbal brands are often preferred for routine use.
